Children's Ministry Authorisation Form

Due to security reasons, please note that this is a once-off exercise. As such, names entered cannot be changed hereafter. If there are amendments to be made, please do so in person at Children’s Ministry or the Level 2 Information Counter.

You may authorise a minimum of one and a maximum of four persons to pick up your child. The full names and identity card numbers of these persons must be filled in.
